Main duties of the job

The Role

  • Deliver high-quality care to our most frail patients and care home residents
  • Provide telephone, online and face-to-face consultations across our linked Leicestershire care homes and to our home-bound patients
  • Lead on care planning, reviews, and proactive management for vulnerable patients
  • Play a key role in shaping and improving primary care services

Job description

Job responsibilities

Clinical

Provide clinical care and examinations to frail patients and patients within the practices associated care homes, including clinical administration as required.

Conduct comprehensive assessments of patient frailty levels and develop individualised care plans addressing frailty concerns, collaborating with healthcare professionals to implement personalised interventions.

Prescribe treatment including medications as a Non-medical Prescriber.

Clinical Governance

Contribute to Clinical Governance- reporting any Learning events, Significant Events, Complaints, or safeguarding concerns.

Conduct regular audit of practices and protocols to ensure compliance, negotiating changes and introducing service improvements as required.

Support the Registered Manager in compliance and targets for the practice.

Leadership and Collaboration

Provide clinical leadership, support and guidance to the practices associated care homes and to other healthcare professionals internally involved in the patients care.

Be a key team player and the go to person for clinical issues for care home patients.

Coordinate interdisciplinary teams, including nurses, caregivers, therapists, and administrative staff, ensuring cohesive and efficient care delivery.

Actively engage in MDT meetings.

Act as an ambassador for the practice.

Professional Development

Maintain professional practice through continuing education, professional updating and when/where available, involvement with professional specialist groups.

Participate in own annual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

Adhere to NMC guidelines for continuing professional development and practice and ensure revalidation criteria is met.

  • Qualified/working towards appropriate Masters level qualification in Advanced Practice.
  • Independent Prescriber Qualification.
  • To work within the regulatory requirements, codes and guidance of the Nursing and Midwifery Council.
