Job summary
We are seeking an experienced and highly motivated Lead
Pharmacist to join our Primary Care team. This pivotal role involves
providing strategic leadership and clinical expertise to enhance medicines
optimisation and deliver high-quality, patient-centred care across a network of
GP practices.
The successful candidate will work collaboratively with GPs,
practice staff, clinical pharmacists, and wider healthcare professionals to
improve outcomes, reduce medication-related risks, and support the effective
use of medicines within local populations.
Main duties of the job
Key Responsibilities:
Clinical Leadership & Patient Care
- Lead and develop safe, effective, and evidence-based prescribing practices across the Group.
- Conduct structured medication reviews for patients with long-term conditions, polypharmacy, or high-risk medication profiles.
- Provide clinical support in managing repeat prescriptions, medication reconciliation, and medicines use reviews.
- Identify and manage patients at risk of medicines-related harm, hospital admissions, or poor adherence.
Strategic & Operational Responsibilities
- Contribute to the planning and implementation of medicines optimisation strategies aligned with national and local healthcare priorities.
- Drive quality improvement initiatives related to prescribing, clinical governance, and patient safety.
- Line manage, mentor, and support a team of clinical pharmacists.
- Act as the point of contact for all pharmacy-related matters within the Primary Care setting.
Collaboration & Training
- Work alongside GPs and multidisciplinary teams to develop integrated care pathways for chronic disease management and preventative care.
- Deliver training and education to healthcare professionals on safe and effective use of medicines.
- Engage with community pharmacy services to improve care continuity and reduce duplication.
- Attend clinical huddles to support wider clinical team.

- Masters degree in Pharmacy (MPharm) or equivalent.
- Registration with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- Significant post-registration experience in clinical pharmacy, ideally within a Primary Care or GP setting.
