We are looking for a talented Nurse (equivalent band 6) with experience of leading the delivery of complex care into the home who would like to work flexibly on our Bank, providing support where required to our new and existing Clients and the Clinical team, under the direction of a Senior Clinial Lead. You will support the team to deliver personalised client-centred service outcomes, carrying out care assessments, writing care plans and risk assessments and assessing competencies.
