Summary:
The purpose of this post is to screen
ADHD self assessment review submissions and provide ADHD support services to
patients. You will work remotely but as part of the Review Team. You will
identify and escalate areas of care that will need escalation or interpretation
from a ADHD Review Clinician.
To work as part of the Review Team to
develop, deliver and contribute to the high quality and efficiency of the
Review Team at ADHD360.
A key part of the post involves screening
the self assessment submission from patients and ensuring the content is
appropriate and relevant. You may need to liaise with patients and families to
clarify any information that is not clear or requires further details.
Working within the Review Team the role
requires screening patient completed screening forms containing information
such as medication, any side effects, physical health/mental health or any
other relevant patient information. Some of this information may require
discussing these details with the patient and gathering further information or
clarity.
The role will require identifying aeras
of concern to raise within the team for attention and action by the prescribing
clinician. This is an autonomous role which involves working remotely with
clear guidelines to follow with a consistent, cohesive and collaborative way
following pathways and processes that will ensure that there high-quality
screening and annual review care for those within our services at ADHD-360.
Working flexibly/remotely across all the
ADHD 360 functions to ensure our patients receive a high-quality review
service. Acting in a liaison role as required and as part of the Review Team
ensuring that screening checks are completed for all our patients requiring a
review. You will work collaboratively to set up strategies for data collection
that is both qualitative and quantitative. Engaging with patients as required
to identify and service/process improvements.
Essential
Duties and Responsibilities:
Carrying out the first screen review of
all patients at this stage.
Identifying any patients screen thar
requires further information or clarification.
Assist in the efficiency on the Review
Team.
Follow the process of the Review Team to
ensure that patients receive a high-quality annual review in line with company
values.
Support the Review Team Manager to
produce reports on the Review Team.
Identify when a patient requires further
clinical intervention and prioritisation within the Review Team.
Assess, plan, implement and evaluate care
safely and effectively, in partnership with medical team and nurses, service
user and carers, using evidence-based practice.
Manage and assess risk within the areas
of responsibility, ensuring adequate measures are in place to protect self,
colleagues, and patients.
Act as per process in healthcare issues
related to the care of the patient which can often be complex and urgent.
Prioritise health problems and intervene
appropriately to assist the patient in complex, urgent or emergency situations,
including safeguarding concerns and act accordingly.
As part of the team provide regular
updates, feedback, and reports to senior management on progress and share any
identified barriers that may need to be addressed in order to meet team
objectives targets.
Report any observed changes in a
patients condition or behaviour, or any other cause for concern, to an
appropriate clinician.
Identify and arrange any requirements for
interpreters for the review appointment with a prescribing clinician.
Refer to colleagues or other agencies as
appropriate, this will include any identified concerns around risks, health
inequalities and safeguarding.
To participate in regular clinical
supervision sessions to receive professional support and time to reflect upon
practice.
Take responsibility for own development,
learning and performance including participating in clinical supervision and
training as necessary to update skills and knowledge and maintaining a record
of own personal development.

Qualifications and Experience:
Qualified
Nurse with current registration. Post 6 months qualification desirable. Demonstrable
ability to connect with and provide support adult and child patients
Familiarity
with EPR systems
Willingness
to learn and follow clinics standards of practice
Excellent
communication skills, ability to communicate effectively with staff and
patients
Sufficient
IT skills to be able to undertake a remote post with virtual assessments
Ability
to work alongside others within a multi-disciplinary team setting
Professional
approach consistent with regulatory body expectations
Excellent
time management and ability to manage own workload
An ability to work on own initiative yet be a key
part of a team
