Paramedic working in a Primary Care Home Visiting service

Job summary

Working within a Primary Care Home Visiting Team which includes other clinicians namely , GPs , ANP's , ECPs and Paramedics you willprovide high-quality, patient-centered care by undertaking home visits for housebound or vulnerable patients with acute, sub-acute, and chronic conditions. The role supports GPs and the multidisciplinary team (MDT) in delivering timely assessments, diagnostics, treatment, and referrals, reducing hospital admissions and improving access to care closer to home.

You will also work in partnership with other acute health care providers as part of the Derby and Derbyshire Local Navigation Hub model .

Main duties of the job

  • Clinical Assessment & Diagnosis
    • Act as an autonomous clinician, performing comprehensive history-taking, physical examinations, and clinical assessments.
    • Diagnose and manage undifferentiated and complex conditions within scope of practice.
    • Undertake and interpret diagnostic tests (e.g., ECG, blood tests, urinalysis).
  • Treatment & Care Planning
    • Provide urgent and routine care, including prescribing (if qualified) or recommending treatment plans.
    • Initiate referrals to GPs, community services, or secondary care as appropriate.
    • Support anticipatory care plans and chronic disease management for housebound patients.
  • Home Visiting
    • Respond to same-day visit requests for acute illness, minor injuries, and exacerbations of chronic conditions.
    • Ensure safe and effective care delivery in patients homes, considering social and environmental factors.
  • Collaboration & Communication
    • Work closely with GPs, nurses, pharmacists, and social care teams.
    • Communicate effectively with patients, carers, and MDT members, including sensitive information.
    • Promote self-care and provide health education to patients and families.
  • Documentation & Governance
    • Maintain accurate clinical records in line with NHS and PCN standards.
    • Adhere to NICE guidelines and local protocols.
    • Participate in audits, clinical meetings, and service development initiatives.

About us

Derbyshire Dales PCN is made up of eleven GP Practices .

The network covers the areas of Hope , Bradwell , Eyam ,Tideswell , Baslow , Bakewell , Darley Dale , Ashover ,Ashbourne ,Brailsford , Wirksworth and Matlock .

There is an excellent road infrastructure and access to and from the M1, A6 and A38 is easily achievable .

You will be working in the beautiful Derbyshire Dales which includes the well known tourist attractions that are scattered around the locality . It is a fantastic place to work with many benefits that are naturally derived by working in the Dales .

  • HCPC registered Paramedic.
  • Experience in urgent or primary care settings.
  • Strong clinical decision-making and triage skills.
  • Ability to work independently and as part of a team.
  • Full UK driving license and access to a vehicle for home visits

Desirable

  • Non-medical prescribing qualification.
  • Experience in chronic disease management and care planning.
  • Familiarity with EMIS System One or similar clinical systems.
