Job summary
Derbyshire Dales PCN are looking to recruit an experienced GP to join our Home Visiting
Service on a permanent contract .
The HVS will also support the ethos of the Derby and Derbyshire ICB Local Navigation Hub model which is currently being rolled out across the county .
You will be required to work sessions across the areas that the DDPCN HVS supports.
DDPCN works in partnership and
provides the home visiting service (Monday to Friday) for the eleven GP
Practices within our locality namely:
Evelyn Medical Centre, Hope
Eyam Surgery
Baslow Health Centre
Peak and Dales Medical Partnership (Bakewell &
Tideswell)
Darley Dale Medical Centre
Matlock and Ashover Group Surgery
Lime Grove Medical Centre.
Hannage Brook Medical Centre
Ashbourne Medical Practice
Henmore Health .

Main duties of the job
As part of the HVS Team you will also support the Local Navigation Hub (LNH) as a GP/Senior
Clinician will provide senior clinical leadership and oversight to the delivery
of the Dales Local Navigation Hub. The GP element will play a key role
in supporting the integrated Neighbourhood Team to improve access, navigation
and co-ordination of care, ensuring patients receive the most appropriate and
patient-centered response to their needs in a timely manner using the current
cross organisational and multi-disciplinary approach to the delivery of care
for the Dales residents.
You will improve the co-ordination and navigation of care for patients and
the psychological safety of LNH and community team members to manage complex,
urgent or challenging presentations in peoples own homes:

The network covers the
areas of Hope , Bradwell , Eyam
,Tideswell , Baslow , Bakewell , Darley Dale , Ashover ,Ashbourne ,Brailsford , Wirksworth and Matlock
.
There is an excellent road infrastructure and access to and from the M1, A6 and A38 is easily achievable .

In accordance with the Home Visiting Service (HVS ) rota, as agreed, the post-holder
will make him/her-self available to undertake a variety of duties including
consultations, telephone consultations and queries, visiting patients at home,
checking and signing/authorising repeat prescriptions and dealing with queries,
paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred from other health care
workers within the organisation as part of the LNH model .
Assessing the health care needs of patients with
undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs
of illness
In consultation with patients and in line with current
practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload
and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to
agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat
prescriptions
Prescribing in accordance with the practice prescribing
formulary (or generically) whenever this is clinically appropriate
The post-holder will be required to visit patients in an
emergency if clinically indicated.
The post-holder will undertake fair share of home visiting
sessions per week as allocated.

- Full GMC registration
- National Performers List registration (England), noting that confirmation on this list is adequate for DBS confirmation
- Appropriate defence indemnity (MPS/MDU)
- Evidence of current validation
- Evidence of last appraisal
- Eligibility to practice in the UK independently
