A fantastic opportunity has arisen with Derbyshire Dales PCN (DDPCN), who are looking to recruit an experienced GP or GPs wanting a weekly flexible clinical session pattern to work within the newly formed Derbyshire Dales ( North and Central) Team Up AHVS provision .

DDPCN works in partnership with the GP Practices in the beautiful north and central Derbyshire Dales.

DDPCN are fortunate to work alongside GP practices and other health and Social Care providers in the towns and villages of Hope , Eyam , Baslow , Ashover , Bakewell , Tideswell , Darley Dale and Matlock .

You will be an enthusiastic GP wanting a role that transforms the traditional boundaries of the current home visiting model of General Practice .

This role will be at the forefront of the delivery of home visiting models of care .

As this is an evolving and developing service ,the successful applicant(s) will be able to influence and drive forward the desire of DDPCN and its stakeholders to deliver a high quality of home visiting clinical care .

You will be supported by an excellent team of clinicians and administrative back -office function that includes dedicated Team Up Care Coordinators .

Main duties of the job

Derbyshire Dales PCN are seeking to recruit a team of Community GPs to work within our newly formed Team Up provision . The role will include working in partnership with multi-disciplinary teams providing care to housebound patients and patients in care or residential facilities. This will be a mix of either in person or remotely.

The aim for the service is to provide a holistic approach to acute on-the-day and/or rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

Provide day-to-day clinical leadership for the multi-disciplinary team including debriefing after visits if needed.

Home visiting for clinically complex cases e.g., end of life care, diagnostic uncertainty, complex prescribing decisions

Triage for the Acute Home Visiting service on a Monday to Friday basis

To build relationships across the various multi discipline teams.

Opportunity for involvement in development as the team expands.

Provide holistic comprehensive assessment and enhanced advance planning as part of MDT's.

Maximise best care in the patients own homes to reduce the need for hospital or care home admissions.

Provide end of life care in line with the patients wishes.
