Intake Clinician (remote)

Job summary

As an Intake Clinician you will be responsible completing thorough Initial Needs Assessments, carrying out comprehensive risk assessments, and making clear clinical decisions about client suitability and pathway allocation. Using clinical judgement, you will triage clients to ensure they are on the correct diagnostic pathway and identify when alternative support is more appropriate.

The role includes actively managing and escalating risk, documenting high-quality clinical information, and providing a professional, compassionate first point of contact while upholding consistent, evidence-based practice across the service.

Main duties of the job

Clinical triage

  • Undertake clinical assessments to gather relevant presenting information.

  • Identify risk factors, safeguarding issues, and mental health concerns.

  • Determine appropriate care pathways.

  • Provided clear clinical rationale for decisions.

Risk assessment and management

  • Conduct risk screenings covering self-harm, suicide, safeguarding and risk to others.

  • Take immediate action where risk is identified, following escalation and safeguarding protocols.

  • Develop safety plans where appropriate.

  • Document and communicate risks clearly to relevant internal teams to ensure continuity of care.

Pathway knowledge and navigation

  • Maintain up-to-date knowledge of internal pathways and suitability criteria.

  • Provide appropriate recommendations based on clinical need.

  • Identify factors that require MDT or Senior Clinician review.

Teamwork and collaboration

  • Attend and contribute to weekly team meetings, fostering collaboration and knowledge sharing.

  • Participate in MDT discussions and handovers.

  • Liaise with external partners and services for onward referrals and information sharing.

  • Collaborate across departments to ensure coordinated, person-centred pathways.
