ProblemShared is seeking an experienced Lead Pharmacist to join our growing prescribing team. Main duties of the job

This is a role for a motivated, enthusiastic prescribing pharmacist, with extensive clinical skills in the field of ADHD and leadership. You will be flexible, solution-focused, and with a growth mindset. You will have experience in prescribing for adults and children. The Lead Pharmacist will support the Clinical Lead in the safe and effective delivery of care within the centralized prescribing duty team, and wider ADHD team. This role is integral to ensuring a consistent and efficient medication service, thus high standards of clinical care and efficient operation of our digital healthcare services, within Problem Shared. You will have a leadership role which will include clinical work, and line manage the prescribers in the prescribing team. You will have a service improvement role within the prescribing team, to improve pathway efficiency and client experience. The Lead Pharmacist will work closely with the Team Leads, Clinical Lead and medication operations team to maintain high standards of care and service delivery.

Job description

Job responsibilities

Support the development of a centralised prescribing model of working within the duty team

Print and send prescriptions which have been electronically created by prescribers

Provide a second line check of prescribing decisions when issuing prescriptions

Support the duty team in responding to client queries, creating and sending repeat prescription requests and monitoring duty ticket requests

Reviewing physical observations submitted by clients and discussing results with clients and/or the wider prescribing team

Escalation of issues, concerns or abnormal results

Line manage prescribers in the duty team

Provide pharmacological advice and in supervision settings and support the wider team

Receiving and posting prescription pads to prescribers and logging prescription numbers

Monitoring risk during contact with clients

MDT or liaison with the specialist advisory group (cardiologist, psychiatrists, paediatrician and specialist ND expert)

Interviewing new candidates.

Personal Development/ training.

Providing safeguarding and risk advice and support to colleagues.

Coordinating the centralised prescribing and duty team meetings and agendas.

Ensuring delivery of evidence based high quality care in line with national guidance

Quality Check:

Perform monthly quality checks on 2% of clinical records, within the centralised prescribing and duty team, to maintain high clinical standards.

Identify areas for improvement and provide constructive feedback to clinicians.

Assessment and Evaluation:

Daily meeting with duty team to allocate workload

Attending team meetings, supervision and CPD to ensure ongoing competency and compliance with standards.

Team Support:

Provide clinical pharmacy support to team members

Meetings and Communication:

Attend weekly team meetings to discuss updates, address concerns, and foster team collaboration. These should be driven by the standardised agenda

Attend weekly meetings with the Clinical Lead to report on prescribing clinic performance, discuss strategic initiatives, and align on goals.

Additional Responsibilities:

Maintain up-to-date knowledge of best practices and industry standards in digital healthcare, in your specific field.

Collaborate with other team members and departments to ensure integrated and coordinated care.

Escalate any issues or conflicts within the team promptly and effectively.

Ensure compliance with all relevant regulations and policies.
