Main duties of the job

We are looking for an experienced Learning Disabilities Assessment Manager to join our innovative Transformation team at Liaison Group.

In this role, you will lead clinical assessments, review medical records, and evaluate individual care needs to determine the appropriate level of support and funding. You will be responsible for redesigning care packages, commissioning and implementing tailored support plans, and developing robust Key Performance Indicators (KPIs) to guide future monitoring.

Working closely with Local Authority partners, you will ensure their input is integrated into care planning and decision-making. This is a pivotal role for a professional with strong clinical judgement and a commitment to improving outcomes for individuals with learning disabilities.

Job description

Job responsibilities

  • Conduct Comprehensive Assessments

    • Gather and review relevant documentation prior to assessment, including medical records and care plans.

    • Undertake clinical assessments involving individuals, their representatives, care providers, and Local Authority professionals.

  • Evaluate Needs and Eligibility

    • Assess individuals ongoing health and social care needs, considering medication, daily living activities, and provider input.

    • Determine eligibility for Continuing Healthcare (CHC) funding, including delineating between health and social care responsibilities.

    • Make evidence-based recommendations, incorporating the views of Local Authority representatives.

  • Provide Clinical Expertise

    • Offer clinical advice throughout the assessment, care planning, and review processes.

    • Ensure care packages are designed to meet assessed clinical needs and are structured with appropriate levels of support.

  • Case Review and Monitoring

    • Regularly review cases to confirm continued eligibility and that support remains aligned with clinical need.

    • Develop and apply Key Performance Indicators (KPIs) to monitor care package effectiveness.

  • Care Planning and Documentation

    • Complete all assessment documentation accurately and in a timely manner.

    • Ensure care plans are updated in line with assessment outcomes and that Local Authority input is fully reflected.

    • Draft clear recommendations for submission to the Integrated Care Board (ICB), supported by robust evidence.

  • Support Individuals and Families

    • Provide information and guidance throughout the assessment process.

    • Respond to complaints with detailed evidence and a professional, person-centred approach.

  • Collaborative Working

    • Work closely with multidisciplinary teams including healthcare providers, local authorities, and commissioning colleagues to ensure joined-up care planning and delivery.
