Main duties of the job
We are looking for an experienced Mental Health Manager to join our innovative Transformation team at Liaison Group. This is a fantastic opportunity to work remotely while leading complex clinical assessments, designing bespoke care packages, and shaping the future of community-based mental health care.
In this role, you will take the lead on conducting Section 117 aftercare assessments, ensuring the appropriate level of clinical and social support is in place for individuals transitioning from inpatient mental health services. You will work collaboratively with Local Authority partners and healthcare providers to redesign care packages and establish clear KPIs to monitor their effectiveness.
You will also play a key role in preventing hospital readmissions by creating tailored support plans backed by strong clinical evidence ensuring every recommendation can stand up to review or appeal.

- Ensure all relevant documentation is gathered and shared with appropriate stakeholders in advance.
- Carry out comprehensive clinical assessments with individuals, their representatives, care providers, and Local Authority staff as needed.
- Assess individual requirements based on clinical records, medication history, care plans, daily living activities, and discussions with providers.
- Offer expert clinical input throughout the assessment, care planning, and ongoing monitoring process to ensure high-quality, needs-led support.
- Reassess current Section 117 cases to confirm continued eligibility and that care packages are aligned with needs and allocated resources.
- Make informed recommendations on eligibility and the appropriate level of care, incorporating input from Local Authority representatives.
- Finalise all assessment paperwork to a high standard, ensuring Local Authority comments are accurately reflected and incorporated.
- Ensure care plans are promptly updated to reflect the outcomes and recommendations of the assessment process.
- Prepare clear, evidence-based recommendations for the Integrated Care Board (ICB) to inform commissioning decisions.
- Provide clear information and compassionate support to individuals and their families throughout the assessment journey.
- Supply robust, evidence-based documentation in the event of complaints or appeals.
