The Care Practitioner role will be part of the Liaison Care team completing Health and social care reviews and Decision Support Tools (DST) producing a recommendation for each case.

The post holder will be undertaking health and social care reviews in Care Homes (Nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards. These will be done with both the provider and the individual (or their representatives) and will be held virtually on MS Teams.

The post holder will also Chair the Multi-Disciplinary Team (MDT) meeting as the Lead Assessor, in relation to the Care Act and the NHS Continuing Healthcare framework Decision Support Tool (DST) and make evidence-based recommendations for eligibility.

You will need to ensure all care plans are up to date and support the client's current needs. You will act as an advocate for the vulnerable, challenging poor practice and discriminatory attitudes and behaviour relating to their care, including both challenging situations at the time and reporting it to an appropriate person.

Main duties of the job

  • Chair Multi-Disciplinary Team (MDT)meetings
  • Completing NHS Continuing Health Care Decision Support Tools with clear eligibility recommendations
  • Completing Care Act reviews and care plans
  • Handle inbound and outbound calls, in an effective and efficient manner, with a focus on building rapport.
  • Manage any relevant follow up calls or requests for further documentation.
  • Conduct the review, supported by a template.
  • Write report of the review using prepared template
  • Ensure reviews are carried out in a timely manner.
  • Always protect sensitive data & maintain confidentiality.
  • Identify and respond to any Safeguarding concerns in a timely and professional manner.
  • Ad - hoc administration duties as specified by Liaison Care Team

Due to a period of significant growth, we are looking for multiple Continuing Health Care Practitioners who want to be part of a vibrant team who are making a difference. We particularly want to talk to people who are Registered Nurses to complement our other qualified team members.

Our team are home based and work remotely on projects around the country.

At this stage we are only seeking applications from candidates who hold CHC experience including experience using a Decision Support Tool.

Main responsibilities

Working within the National Framework for NHS Continuing Healthcare (CHC) and / or the Care Act 2014 you will chair Multi-disciplinary Meetings (MDTs), and complete reviews and assessments on behalf of Integrated Care Boards (ICB). These will be done with both the individual, the provider and their family (or their representatives) and will be held virtually on MS Teams or Zoom.

Experience and qualifications required

Essential:

  • Registered Level One Nurse with NMC
  • 12 months post qualifying experience
  • Operational Continuing Healthcare experience with completion of Decision Support Tools (DSTs) and or CHC reviews.
  • Administration, Clerical experience
  • Computer literacy, specifically Microsoft suite to include Outlook & Excel
  • Knowledge of CHC National Framework 2022, Care Act 2014 and Mental Capacity Act 2019

Desirable:

  • Experience in working to Key Performance Indicators (KPIs)
  • Project management experience
  • Working in multidisciplinary environments
  • Person centred care and support planning
  • Experience of working with Personal Health Budgets and Direct Payments
