Liaison Care are seeking their next Delivery Manager to work alongside the other Delivery Managers to lead the teams delivering Continuing Healthcare and Adult Social Care programmes of work, services and associated programmes.

Main duties of the job

The role will be to work alongside the other Delivery Managers to lead the teams delivering Continuing Healthcare and Adult Social Care programmes of work, services and associated programmes.

You will be responsible for the management of a committed and diverse team consisting of social care, nursing, and administrative professionals. You will have direct line management responsibility for our Delivery Leads who are aligned to programmes of work.

You will oversee the delivery of specific client programs and engage confidently with Clients on a regular basis.

You will be a member of the Delivery Management Team and key to the growth of the organisation, which is currently on a steep trajectory.

You will be a champion for quality, putting the individual at the heart of what we do. You will have responsibility for the quality assurance of program work in line with the CHC (Continuing Health Care) National Framework 2022 and Care Act 2014 and our high client service requirements.

You will be a qualified Nurse, Allied Health professional or Social Worker and be currently registered with your professional registration body.

Principal tasks and activities:

To lead the delivery teams and line management of the Delivery Leads.

To ensure that team members are supported and developed to reach their potential.

Undertake and support audits relating to all functions across Liaison Care.

Work closely with the COO and other members of the Delivery Management team to improve service offerings and business efficiencies.

Support a proactive and inclusive culture, leading by example.

Development and review of policies, systems, and processes around CHC, 1-1 Care, Fast Track, Adult Social Care and Joint Funded packages of care linked to the relevant key national and local priorities.

Development of strong clinical and social care evidence-based governance processes, including appropriate input to aid protection of vulnerable adults and children.

Support the development of local policies and processes, and long-term strategic plans. Communication and Relationships Engaging with teams to provide updates on business growth, performance, and priorities.

Ensuring effective communication and liaison with key stakeholders, including the individual and their families or representatives, ICB (Integrated Care Board) colleagues, local authority professionals, solicitors, and private and statutory care providers.

Responsibility for ensuring individual / carer involvement at all stages of the Liaison Care processes.

Promote excellent customer care throughout the Liaison Care process, ensuring local resolution of any concerns where possible.

Knowledge, Training and Experience

Specialist knowledge across a wide range of work procedures and practices related to CHC, Adult Social Care Assessment and Reviews, 1-1 funding reviews, knowledge of the Mental Capacity Act, and Deprivation of Liberty Safeguarding.

Provision of specialist subject matter expert advice, as necessary. Non-clinical advice includes information on national and local policies and strategies, including Adult Social Care development and requirement

Assimilate national policy and guidance, interpreting the local implications, cascading, and ensuring implementation. Analytical and Judgemental

To work within performance management data to ensure effective monitoring of activity, analysis, and contract compliance.

Chair Local Resolution Meetings, specifically CHC resolution meetings where these arise Planning and Organisational

Responsible for ensuring that robust systems are in place to assess and review an individuals care and support needs.

Responsible for ensuring consistency in the application of national and local policy on eligibility for CHC and Adult Social Care

Identify and respond to changing priorities within own workload and support others to do the same.

Devise and implement programmes to manage resources and workload demands.
