Job summary

We are looking for an experienced dietitian to provide maternity cover within a Primary Care Network in Armley, Leeds. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices.

As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community.

Main duties of the job

You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, and frailty - helping them make lasting changes through person-centred care and behaviour change approaches.

You will also have opportunities to contribute to service improvement, clinical audit, and pathway development. Group education sessions may form part of the role depending on service needs.

Job responsibilities

Clinical Responsibilities

You will work autonomously within GP practices, managing your own clinical caseload of patients with a wide range of nutrition-related conditions. This includes:

  • Assessing, diagnosing, triaging, and developing management plans for patients presenting with pre-diabetes, type 2 diabetes, weight management needs, frailty, malnutrition, and other dietetic conditions commonly seen in primary care
  • Providing personalised, evidence-based care plans that take into account each patient's clinical status, lifestyle, preferences, and personal goals
  • Supporting patients to make sustainable behaviour changes and develop confidence in self-management, reducing reliance on pharmacological interventions where appropriate
  • Working with patients who have complex needs, including those with psychosocial factors or mental health considerations, and adapting your approach accordingly
  • Referring patients to other services when appropriate, including secondary care, social prescribing, and community support, working within local referral pathways
  • Delivering consultations via a mix of face-to-face, telephone, and video appointments depending on patient need and service requirements

Multidisciplinary Team Working

You will be an integral part of the Primary Care Network team, working collaboratively to deliver joined-up patient care. This includes:

  • Building strong working relationships with GPs, practice nurses, clinical pharmacists, social prescribers, health coaches, and other PCN staff
  • Contributing to reducing GP workload by managing dietetic referrals directly and providing specialist input within the practice team
  • Improving patient access to timely dietetic care within the community
  • Participating in MDT meetings and case discussions where relevant

Service Development, Audit and Quality

You will play an active role in shaping and improving the service. This includes:

  • Supporting audit, evaluation, and outcome monitoring to demonstrate service impact and identify areas for improvement
  • Contributing to the development of efficient, streamlined care pathways within primary care
  • Identifying opportunities to enhance patient experience and clinical effectiveness
  • Supporting the implementation of new initiatives or service developments as required
Professional Development and Governance

You will be supported to maintain high professional standards and continue your development. This includes:

  • Practising in line with HCPC standards, professional codes of conduct, and organisational policies
  • Engaging in regular clinical supervision and reflective practice
  • Participating in training opportunities, including the HEE Roadmap to Practice programme

  • Previous clinical experience of first contact triage clinical work; even if in the private sector or freelance
