We are seeking a dedicated and versatile dietitian to join our team and work in-person with a Primary Care Network (PCN) in Nottingham. The ideal candidate will be a generalist dietitian with a strong background in frailty, diabetes, and weight management. About Us:
We are committed to advancing the careers of dietitians by helping them become First Contact Dietitians (FCDs). As an FCD, you will be a qualified autonomous clinical practitioner working directly within GP practices. You will be empowered to assess, diagnose, treat, and manage dietetic issues and undifferentiated conditions. In many cases, you will have the authority to discharge patients without the need for a medical referral.
Main duties of the job
- As a Dietitian within the PCN, you will be a skilled practitioner capable of operating at an advanced level of clinical practice. You will bring comprehensive knowledge and expertise in managing a wide range of dietary issues and conditions, contributing as an essential member of the multi-disciplinary team (MDT) within the PCN.
Key Responsibilities:
- Clinical Practice:
- Provide expert, person-centred dietary interventions across the full patient care pathway, from initial clinical assessment and diagnosis to treatment and ongoing evaluation.
- Apply advanced clinical skills to develop individualised care plans that address the specific dietary needs of patients within key groups, including those with diabetes, weight management challenges, and frailty.
- Patient Support:
- Work across multiple GP practices within the PCN to support patients in primary care settings, ensuring continuity of care and accessibility.
- Specialist Areas:
- Utilise your knowledge and expertise in diabetes management to provide comprehensive dietary support for patients living with this condition.
- Provide tailored nutritional support for frail patients, focusing on maintaining and improving their overall health and well-being.
This role demands a proactive approach, with the ability to work independently and as part of a collaborative team, ensuring that each patient receives the highest standard of care.

- Assess, diagnose, triage, and manage patients, taking responsibility for the management of a caseload
- Develop and make use of your full scope of clinical practice, including skills relating to supplementary prescribing and investigation when trained to do so
- Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate
