Job summary

Lead, motivate, and manage team of care professionals, fostering a positive and collaborative work environment.

Conduct regular supervision, training, and performance evaluations to enhance staff competencies and ensure highest standards of care.

Oversee the development and implementation of personalised care plans in collaboration with the clients, their families and healthcare professionals.

Conduct comprehensive assessments ,considering physical, emotional,and social aspects and adapt care plans to address evolving needs.

Build strong relationships with clients and their families, actively involving them in the care planning process and addressing any concerns or preferences.

Main duties of the job
