Job Title: Patient Safety Investigator - Band 6 Department: Clinical Governance Salary: £45,469 - £55,564 Location: The London Clinic, 1 Park Square West, Marylebone, London Job type: Permanent Hours: 37.5, Monday to Friday (Hybrid with 3 days onsite) About Us: The London Clinic is one of the UK's largest and most prestigious independent hospitals, located in the heart of Marylebone. The Role We are recruiting a Patient Safety Investigator whose primary function is to carry out thorough reviews of patient safety incidents and ensure that learning is captured and acted upon. The focus of this role is to understand what happened, why it happened and what needs to change to prevent recurrence. You will lead and support patient safety incident investigations from start to finish. This includes gathering evidence, interviewing staff, analysing clinical information, identifying contributory factors and producing clear investigation reports. Your work will directly support our Patient Safety Incident Response Framework and strengthen the way we learn from incidents across the hospital. The post holder will work closely with clinical and non clinical colleagues to test assumptions, challenge practice where needed and ensure that improvement actions are realistic and measurable. You will also support communication with patients and families, and provide assurance to regulators and insurers that our investigations are robust and transparent. Applicants must hold current professional registration with a UK regulatory body and must already understand clinical governance, PSIRF methodology, Datix processes and the principles that underpin safe care. Candidates without this background will not be suitable for the role. Key Duties:
Lead and support investigations that are timely, proportionate and aligned with PSIRFApply systems-based approaches and human factors principles to identify contributory factorsGather and analyse evidence including statements, interviews and clinical recordsProduce structured investigation reports that offer clarity, assurance and learningReview Datix data to identify themes and support improvement planningSupport clinical teams with monitoring and delivery of improvement actionsContribute to patient safety education across the organisationEngage with patients, families and regulators with professionalism and sensitivity Ideal Candidate This role suits a registered clinician who is ready to move from direct clinical practice into a patient safety and governance focused career. You will be confident examining complex events, asking direct questions and challenging assumptions when required. Strong communication, attention to detail and an analytical mindset are essential. Skills and Experience:
Current registration with a UK professional body such as NMC, HCPC or GMC.Clear knowledge of patient safety principles, the national patient safety framework and the fundamentals of clinical governance.Understanding of PSIRF and ideally experience supporting or delivering PSII investigations.Awareness of Datix processes and the use of incident reporting systems.Experience in patient safety, quality improvement or clinical governance within an acute healthcare setting.Relevant clinical experience in a hospital environment.Experience conducting or supporting investigations into patient safety incidents.Strong analytical skills and the ability to interpret data and identify patterns.Skilled in producing clear and concise reports for assurance purposes.Confident communicator with the ability to engage with patients and staff sensitively and professionally.Competent in the use of Microsoft applications including Word, Excel and PowerPoint.
