ABL Health delivers an integrated wellbeing service on behalf of Torbay Councils Public Health Team, bringing together health coaching, Tier 2 weight management, and stop smoking support to improve the health of local communities. The service focuses on accessible, community-based delivery, offering health education, training, and tailored support to residents of Torbay, those working in the area, and individuals registered with a Torbay GP.

Main duties of the job

We are looking for a motivated and adaptable individual to deliver ABL Healths Healthy Lifestyles service across the community, supporting clients through one-to-one sessions, group programmes, and digital platforms such as Zoom, Teams, and telephone consultations. The successful candidate will provide evidence-based education on nutrition, physical activity, and behaviour change, using a client-centred approach that incorporates positive psychology and motivational interviewing techniques. This role involves regular travel across Torbay to reach priority communities, monitoring client progress through consultations and accurate data recording (including use of Microsoft Excel), and tailoring interventions for those not on track to meet their goals. You will work collaboratively with clients and stakeholders to co-produce session materials, contribute case studies for performance reporting, and support social media engagement. Additional responsibilities include assisting with training new staff and volunteers, recruiting participants at community events, maintaining detailed and compliant records, and ensuring adherence to ABL Health policies and clinical governance standards. The role also requires strong organisational skills to manage workload priorities, active participation in team and multidisciplinary meetings, and a commitment to continuous professional development and service improvement.

The focus of the service is to deliver community-based provision, deliver health education and training and a core service of Tier 2 weight management provision and supporting residents of Torbay, those who work in Torbay or those registered with a Torbay GP to stop smoking.

You will be crucial in delivering our programmes to the local community - including weight management, smoking cessation and other well-being schemes. You will work with this community to develop services that meet their needs. The ideal candidate will be passionate about delivering client-centred motivational change and supporting people living with long term conditions to take up healthy lifestyles. You will have a background in working within health promotion, nutrition, exercise, health improvement, adult and/or family weight management, smoking cessation or similar and be an excellent team player and have a willingness to learn and develop.

Duties and Responsibilities

Deliver ABL Healths Healthy Lifestyles service, through one-to-ones or group sessions in the community, and through digital methods such as Zoom/Teams or phone appointments.

Deliver evidence-based education around nutrition, physical activity and behavioural change as advised by ABL Health team and in line with ABL Health programme curriculums.

Regularly travel across Torbay to deliver the service to areas of need.

Deliver programmes with a client-centred approach using skills of positive psychology and motivational interviewing.

Monitor client's progress through regular consultation and with the use of Microsoft excel, identifying specific treatments for those not on track to achieve their goals.

Work with clients and stakeholders to co-produce in-session materials

Contribute to the contract performance report by submitting regular case studies to the project lead.

Contribute to and support ABL Healths social media pages.

Contribute to training and support new staff and volunteers.

Working with individuals and groups using SMART goal-setting.

Keep detailed files, recording data accurately, ensuring all forms and other documentation is completed correctly.

Have a thorough understanding of ABL Health policies and procedures.

Engage and recruit potential clients at promotional and community events.

Contribute to ongoing service development needs.

To attend and actively participate in team and wider company meetings, such as MDT.

Be able to organise own workload in order of priority and deadline dates, daily administration tasks and other work-related activities.

Skills and Competencies Required
