Main duties of the job
The successful candidate will work as part of a team to deliver a weight management programme to clients across Nottinghamshire through telephone consultations, Zoom group sessions and face-to-face delivery. The role involves supporting clients throughout each stage of the programme by providing personalised advice to help them lose weight, achieve their health goals and make sustainable lifestyle changes, while maintaining and managing a caseload of participants. Responsibilities include communicating with clients via telephone, email and secure messaging, reviewing food diaries and programme progress, and escalating cases when additional support or specialist input is required. You will liaise closely with the wider multidisciplinary team through regular MDT meetings, contribute to the development of programme resources and curriculums, and provide ongoing support by responding to client enquiries via phone, email and messaging services. The role also includes attending training and team meetings, representing the organisation at relevant events, handling sensitive data with a high level of confidentiality and attention to detail, and ensuring accurate and timely data entry to maintain a clear and effective client pathway. In addition, the postholder will contribute to the wider integrated wellbeing service, supporting initiatives such as physical activity delivery, family weight management programmes, smoking cessation support and brief advice on alcohol consumption.
