Job summary

Our service delivers adult autism assessments alongside a Navigation Service offering up to 12 weeks light touch support for individuals awaiting an assessment. The successful candidate will use their knowledge and skills to enable adults to reduce specific difficulties associated with autism through non-judgemental support, helping to build confidence and independence that can lead to a better life.

Main duties of the job

As a Navigation Support Worker, you will manage an allocated caseload under the supervision of a qualified practitioner, working closely with a multidisciplinary team. You will consider the physical, psychological, emotional, and environmental needs of individuals referred to the service, supporting them and their networks through person-centred planning. Your role involves independently travelling across the county, facilitating both individual and group psychoeducation sessions, and promoting independence and social inclusion. You will lead the planning and delivery of sessions, adapting content based on client feedback and outcome data, while also supporting service development through co-production and collaboration with other agencies. Providing advice around autism, reasonable adjustments, and wellbeing strategies, you will liaise with clients, their support networks, and professionals, maintaining strong communication and adapting for diverse needs. Additional responsibilities include supporting assessments, contributing to care planning and reviews, modifying self-advocacy tools, and undertaking administrative tasks such as appointment scheduling and referral processing. You will play an active role in quality assurance and risk identification, helping to shape a responsive and inclusive service.

Service Overview: Be U Norfolk

Our service delivers adult autism assessments alongside a Navigation Service offering up to 12 weeks light touch support for individuals awaiting an assessment. The successful candidate will use their knowledge and skills to enable adults to reduce specific difficulties associated with autism through non-judgemental support, helping to build confidence and independence that can lead to a better life.

Main Responsibilities:

To work with an allocated caseload under the supervision of a qualified practitioner and in close collaboration with the multi-disciplinary team.

To have regard for the physical, psychological, emotional and environmental needs of the individuals referred to the service and their support network.

To support in the day-to-day running of the navigation service by travelling across the county to venues convenient to the client independently (lone working duties form part of the role).

To take the lead in planning, organising and facilitating sessions for clients.

To support ongoing service development through facilitating co-production opportunities.

To monitor the information presented and take responsibility for ensuring that it is up to date and relevant to the clients attending.

To adjust planning to the service and client needs. This may include identifying, inviting and coordinating other relevant agencies to contribute to action plans.

To monitor and collate the outcome data from sessions and to use this to highlight potential changes and adjust session content accordingly.

To plan and lead group and individual sessions that focus on psychoeducation independently and in conjunction with the case holder.

To provide advice about Autism, reasonable adjustments and management strategies that contribute to the clients wellbeing and education.

To provide advice to other professionals regarding Autism, the challenges that it can cause and strategies for managing these.

To provide advice to the clients support network involved with the pre diagnostic support regarding strategies to support the client.

To promote independence and social inclusion for those referred to the service.

To identify needs and seek support from our Assistant Psychologists or the wider ABL clinical team when required.

To work independently in a variety of settings reporting observations or concerns back according to ABL policy.

To maintain and promote good communication with clients, their support network and professionals.

To maintain an awareness of the clients challenges in communication and adapt to accommodate these.

To develop a knowledge and understanding of the specialist needs of autistic adults without learning disabilities and the approaches that they may benefit from.

To support the assessment and management of individual care needs through initial data collection, monitoring of progress, feedback, discussion and reports and in care reviews.

To modify and progress self-advocacy passports using own clinical reasoning in conjunction with clinicians.

To share responsibility for indirect patient contact tasks such as, answering telephones, arranging appointments, processing referrals making resources and inputting activity data.

To contribute towards the ongoing quality of service and care and identify any risk issues in relation to the clients health and social care need.

To promote and develop a valued lifestyle and independence for clients.
