Job summary
ABL Health delivers the Integrated Health Improvement
Service, Your Health Dudley, commissioned by the Dudley Council Public Health
Team. This transformative service will play a central role in improving the
health and well-being of residents of Dudley. The service will provide
integrated behaviour support to help people move more, drink less alcohol, stop
smoking and lose weight. The service will also offer wellness coaching for
families, deliver community health checks, offer advice and guidance, and
proactively signpost to relevant organisations. All of this is embedded in
sustainable behaviour change and positive well-being. This post will focus on the wellness coaching
for families with school age children.
Main duties of the job
The post holder has responsibility
for delivering the following key result areas:
Increase new referrals to the service and reduce drop out rates
Provide one to one family support, mainly face to face but
sometimes by phone or video call
Work with local schools to encourage parents to engage with the
service
Build on network of signposting opportunities to enrich support
offered
Build local links with other support service staff to encourage
referrals and partnership working
Promote the service at local meetings, networks and events
Work with Service Manager and Coordinator to ensure service is achieving
KPIs
Work with data team to ensure system reporting meets the needs of
the service
Attend ABL forums, meets and training as applicable
Work with marketing to create family friendly booklet material
Work with system in order to extract data for use in reports and
at managers request from ABL meetings and updates
Maintain good working relationship with Family Commissioners
Co-create and deliver healthy workshops for use with families,
such as the Healthy Lunch box session
Group workshop delivery out in community, such as healthy eating
workshops, healthy pizza making and delivering fun and informative nutrition
topics
To safeguard all families and report in line with ABLs
safeguarding policy

- Experience of delivering group work and 1-1s
- Experience in using data (service data, population health data) to inform service development to improve outcomes.

- Knowledge of safeguarding in a community health setting.
