Job summary

ABL Health, North Lincolnshire and North East Lincolnshire ICS deliver a Specialist Tier 3 weight management service alongside our Tier 4 bariatric surgery service at Castle Hill Hospital.

You will be joining an experienced team consisting of a Project lead, Care Co-Ordinator, Nutritionists, Therapists, Healthy Lifestyle Advisors, Exercise Specialists, and Administrators. Your role will be to provide dietetic expertise for the Weight Management Service and work alongside a wider team of professionals such as GPs, Psychologists, Dietitians, Nutritionists, Exercise Specialists, and Healthy Lifestyle Advisors. You will be responsible for providing dietetic patient care, leading dietetic-led clinics, contributing to MDT, and supporting group education and intervention sessions. The interventions can be carried out via telephone, digital platforms, or face-to-face community venues across the footprint. You will be supported by the Professional Lead, Project Lead, and the wider MDT.

Main duties of the job

To provide a Tier 3 Dietetic Weight Management service to help ABL Health to continue to deliver an excellent service to the people living with obesity in North and North East Lincolnshire.

Delivering a person centred intervention for the clients from initial clinical assessment, diagnosis, treatment and evaluation of their care. Demonstrating safe, clinical decision-making and expert care for clients within the service.

To assist the existing team in providing a quality dietetic input within ABL Health. You will work closely with other Nutritionist and members of the MDT for both patient care and ongoing service development. Additionally, you will provide specialist advice within ABL Health and externally to dietetic colleagues, GPs, Nurses, and other Health care Professionals. The role will require dynamic working which embraces the vision and values we wish to establish for the Tier 3 weight management service to support people to achieve healthy lives.

ABLs highly experienced and friendly multi-disciplinary team of Health and Social Care professionals, Business Intelligence, Service Development, Governance, Finance and Business Development are there to support. Additionally, existing experienced delivery teams will be available to support you to drive the Service forward.

Job responsibilities

  • Providing high quality, evidence-based clinical care to support clients in line with ABL Health protocols and professional standards.

  • Deliver one-to-one or group patient consultations to adults, children and young adults via telephone, video calls or face-to-face.

  • Using a wide variety of communication methods with clients, colleagues and other healthcare professionals.

Carry out comprehensive assessments using motivational interviewing techniques and develop and implement an appropriate and agreed treatment plan based on behavioural change skills to ensure long-term positive outcomes.

  • Utilising expert diet and nutrition knowledge to support clients with a range of evidence-based dietary approaches including pharmaceutical and alternative lifestyle interventions.

  • Supporting medically and psychologically complex clients including those with multiple comorbidities using the MDT where needed for additional support.

Maintaining accurate, professional and up-to-date clinical records using the ABL database.

Maintaining adequate CPD and continuing to meet HCPC registration requirements and ensuring the completion of all mandatory training.

Being an active member of the MDT, providing feedback from dietetic assessments into the MDT meetings. Liaising with appropriate health care professionals when needed.

Following the Safeguarding policy and protocol and Incident reporting policy at all times.

Adhering to all Information Governance and data protection regulations in the handling of patient data.

  • Supporting the project lead with the monitoring and management of clinical governance including audits and KLOEs.

  • Be professional and legally responsible and accountable for all aspects of your own work including the management of clients within your care. To ensure a high standard of clinical care for the clients under your management and support nutritional staff to do likewise.

  • Leading clinical supervision meetings with the healthy lifestyle advisors in the service.

Updating the team regarding the specialist dietetic management of clients living with obesity after the publication of new recommendations and after attending conferences, study days, webinars etc.

Work flexibly, including from home or on site, or running an evening clinic, and be able to travel to other areas as and when required.

Skills and Competencies Required

Essential:

BSc in nutrition and dietetics and registered with the Health and Care Professions Council.

Membership of BDA or alternative professional indemnity insurance in place

Keen interests in obesity management with experience of facilitating behaviour change

Organised and have the ability to work on own initiative, to deadlines and demonstrate innovative practise

Excellent verbal and written communication skills.

Be able to demonstrate empathy, tact and diplomacy and skills in behaviour change strategies.

Able to plan and deliver training sessions to patients or other health care professionals.
