Empowered Living Team Coordinator

Job summary

An exciting opportunity has arisen as Empowered Living Team (ELT) Coordinator at St Josephs. You will work as key member of a committed and creative Therapies team, which offers innovative, holistic and person-aligned care and support to people with palliative and end life care needs, and to their families and care-givers.

The post holder will provide coordination for, and service development of, the Empowered Living Service. This community-based service provides outstanding person centred rehabilitative enablement for people living with a palliative diagnosis. Following a joint professional assessment a detailed programme of patient led goals is enabled and supported by a trained ELT volunteer, in the patients own home. Volunteer recruitment, training, patient matching and regular support group/1-1 supervision is led by the ELT coordinator.

Main duties of the job

  • A skilled professional with experience in health and social care, education or community development
  • Experience of multi-disciplinary working in an community rehabilitation settings
  • Experience working in an education or support role in relation to volunteers
The job is based at St Josephs. Some local travel is essential.

Volunteer Support and Management

  • Recruit suitable volunteers
  • Deliver ELT training modules and encourage volunteer personal development
  • Ensure regular clinical supervision is provided for all volunteers individually or as a group
  • Undertake operational management of ELT volunteers, matching them with suitable patients
  • Oversee volunteer and patient matches for the duration of the programme
  • Ensure ELT volunteers are compliant with maintaining accurate and timely records of their contact with patients and carers
  • Record statistics on numbers of referrals and volunteer patient contacts
Clinical Responsibilities

Work with members of the Therapies Team to support volunteers to deliver empowerment plans including:

  • Mobility practice
  • Supporting patients to complete home exercise programs
  • Practising breathing control techniques
  • Promotion of independence whilst maintaining patient safety
  • Recognise changes in patients condition, modifying input with patients where appropriate and reporting back to a physiotherapist in a timely way
  • Proactively seek support from colleagues for complex cases or in situations where a patients condition is changing
  • Be skilled in communication with people who have life limiting illness
  • Maintain accurate and up to date documentation for all patients, consistent with department documentation and Chartered Society of Physiotherapy standards

  • If professionally registered, abide by the rules of professional conduct laid down by the appropriate professional body
  • Identify own learning and development needs
  • Take responsibility for own continuing professional development by attending regular 1-1 meetings with your supervisor and actively seeking to develop skills and knowledge
