Job summary
We are looking for experienced and motivated General Practitioners to join our South West London Clinical Assessment Service (CAS), working either remotely or from our clinical hub in Putney. As part of our integrated urgent care service, you will provide safe, effective and timely clinical assessment to patients who require enhanced triage and expert decisionmaking. You will deliver telephone and online video assessments, emailbased clinical advice and timely onward referral where required. Working within your professional competencies, you will assess, diagnose, plan care and prescribe as appropriate, ensuring patients receive the right care at the right time. You will support the wider urgent care hub by offering clinical expertise to colleagues and contributing to realtime decisionmaking that maintains service safety and efficiency. As a key member of a crossdisciplinary team, you will help shape highquality, evidencebased care and support the development of new service models that positively influence the local health economy. You will also represent Practice Plus Group professionally when interacting with patients, partners and external stakeholders. Main duties of the job
- Providing telephone and online video assessments for patients requiring enhanced clinical triage - Monitoring and managing clinical hub triage queues Assessing a wide range of cases, including: - Children under 5 without a clear Pathways disposition - Adults over 75 without a clear Pathways disposition - Emergency contraception queries - Medication questions and prescription requests - Supporting Clinical Advisors in validating Emergency Department and Green Ambulance dispositions - Navigating appropriate cases for home visiting or district nursing services - Managing prescription requests alongside pharmacists and administrative teams - Offering clinical advice to Mental Health Nurses, ANPs, dental nurses, pharmacists and NHS 111 Clinical Advisors - Providing realtime support to Service Advisors and Supervisors through floor walking - Ensuring accurate documentation of all contacts in clinical systems - Working within protocols, policies and prescribing formularies - Participating in clinical governance, supervision, training and performance review - Maintaining uptodate mandatory training and clinical competencies
