We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in West Yorkshire.

Hours - Monday to Friday; 8am - 6pm Some weekend working may be required

Salary -This position is full time and you will receive a salary of £42,094 to £47,446 depending on experience

Location - West Yorkshire Prison Sites - HMP Leeds, HMP New Hall & HMP Wealstun & Community

Main duties of the job

As a Clinical Reconnect Practitioner your responsibilities will vary. You will  

  • you will work autonomously in prison and community , to identify and respond to requests of screening and assessment of people with identified health needs leaving the prison establishment within a 12-week period or less.  

  • The aim of the service is to reduce reoffending by addressing the health and social needs of the service user and ensuring onward referrals are made to ensure continuity of care provision. This role will work closely with probation and other key stakeholders to ensure the needs of the service user are effectively assessed and addressed, avoiding duplication. 

  • Working autonomously, you will be required to ensure the needs of the service user are met and communicated to relevant parties to ensure continuity of care, onward support and effective risk management.

Qualifications and skills required:

  • Qualified Registered Mental health Nurse, Mental Health or Social Worker with current registration with the relevant professional body.
  • Experience of working with patients, face to face, who have complex mental health conditions 
  • Awareness of substance misuse services and support available 
  • Sound clinical knowledge and assessment skills including risk assessment  
  • Understanding of the complexities of multi-disciplinary and multi-agency working  
  • A non-judgmental and compassionate approach  

Our person-centred model is focused on providing meaningful pathways to anyone faced with criminal charges, as well as understanding what triggers and socio-economic circumstances might lead to people offending. We will treat individuals more holistically, focusing on the person and early intervention strategies to reduce risks and improve outcomes.
