We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP YOI Doncaster, a category B Men's Prison located in Doncaster, South Yorkshire.
In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.
Hours
Monday to Friday (08:00 - 17:00)
Salary
This position is full time, youll receive an annual salary up to £47,466 per annum - depending on experience.

Main duties of the job
As a Long Term Conditions Nurse, your responsibilities will vary. You will
- Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
- You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing
- Facilitate arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care.
- Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.
- Inspire the continuous improvement of MDT working across HMP and healthcare teams.

Qualifications required
- Registered General Nurse
- Knowledge of health and social care models across the criminal justice system and of national and local initiatives
- Knowledge of Evidence Based Practice and Clinical Governance
- Experience of assessing the needs of and working with individuals with complex health and social care needs.
Although not essential ideal applicants would have experience in a hospital, community or prison environment.

to urgently address improvement recommendations from CQC and HMPP inspections
Deliver continuity of care across
health and social care agencies from courts, to custody and community whilst
improving the communication between the above.
Inspire the continuous improvement
of MDT working across HMP and healthcare teams.
Promote patient confidence
about the management of their condition and fosters patient responsibility to
make appropriate healthy choices
Share ways to reduce
demand into other health and HMP resources (e.g. escorts and transfers to
primary and secondary care) whilst improving access to and utilisation of
health or social care resources.
