We have an exciting opportunity for a Release and Transfer Nurse to join our Primary Care team at HMP Brixton, a category C Prison located in Brixton, London.

In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.

Hours

Monday to Friday: 07:30 18:00

You may be required to work some late shifts and weekends

This position is full time working 37.5 hours per week.

Salary

Youll receive an annual salary up to £53,172 per annum - depending on experience.

Main duties of the job

As a Release & Transfer Nurse, your responsibilities will vary. You will

  • manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment
  • provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment
  • work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment

Qualifications & Experience Required

  • Registered Adult Nurse
  • Evidence of implementation of local and national policies e.g. NICE guidance
  • Experience demonstrating high levels of clinical expertise
  • Excellent communication skills and ability to communicate openly, consistently and clearly
  • Flexible attitude to working arrangements

Although not essential ideal applicants would have experience in a hospital, community or prison environment.

Job responsibilities

In order to meet the needs of patients in our prisons, Practice Plus Group have developed a new integrated approach to the provision of care. Teams will work together at each stage of the patient journey, supporting them as they arrive in the prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team).

As Release and Transfer nurse you will use your expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems. You will provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment. You will be expected to work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment.

The Release and Transfer nurse will conduct a holistic patient assessment to include:

      • Clinical
      • Psychological
      • Substance use
      • Social
      • Education, training & employment
      • Spiritual/chaplaincy

You will manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment.

Management of key risks associated with resettlement include:

Disengagement/apathy towards recovery due to length of stay in the justice system.

Anxiety related to housing, relationships and financial concerns.

Newly sentenced, resulting in increased suicide /self-harm risk

Detoxing on transfer

Mental illness

Poorly managed challenging behaviours.

Attend a multi-disciplinary pre-release meeting that includes representatives from safer custody, healthcare and external services.

Liaise with social care providers

Liaise with other establishments on receiving and send out patients for their transfer.

Home Office detention cases

Your role will consist of managing face to face clinics with patients and being the experienced clinician for patients who may need a little more support for their release/ transfer. You will work closely with GP colleagues for those cases that need further nursing input. You will request TTOs for those patients leaving the establishment and ensure they have a discharge letter and/or letters for any future hospital appointments they may have

You will also;

Provide outstanding evidence based care, listening and acting on patient feedback. Ensuring that care is personalised and informed by what matters to them. You will also provide assessment, diagnostics, treatment and continued management of patients with acute and long term medical conditions.

Deliver non-judgemental care by leading the team to deliver care that is safe, high quality, patient centred whilst meeting CQC requirements following our policies, procedures and always considering safeguarding.

Inspire excellence in your colleagues by pro-actively leading and motivating the team recognising that our workforce is our most valuable asset. You will provide professional leadership to the nursing team to ensure we provide a service that people can trust, feel safe within and feel proud of.

Promote best practice, clinical supervision, evidence based care and continuous quality improvement embedding a culture of reflection and shared learning.

Share knowledge, skills and expertise to ensure safe care and support your clinical and non-clinical colleagues to do a great job.
