We have an exciting opportunity for a Transitional Worker to join our team at HMP Wormwood Scrubs, a category B Prison located in London.
Hours
Monday to Friday - 8am to 4pm
Salary
This position is full time, youll receive an annual salary up to £33,232 per annum - depending on experience.

Main duties of the job
As a Transitional Worker your responsibilities will vary. You will
- hold a case load of complex patients and support them with arranging up to 3 months support in the community
- be support in attending hospital appointments, housing or employment
- attend the employment hub and triage men before they are seen in the pre-release clinic
- link in with wellbeing organisations in the community who can take on referrals for extra support

Qualifications and experience required
- English and Maths at GCSE or equivalent
- Ability to prioritise workload
- Excellent communication skills
- A non-judgmental and compassionate approach
- Good IT skills and IT literacy
- Ability to work within a busy environment
- Experience of being a support worker in prison or the community
