We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Bullingdon, a category B Prison located in Bicester, Oxfordshire.
HMP Bullingdon is a male remand prison that has a capacity for 1,152 the offers 24 hours healthcare and there is an inpatient unit on-site.Hours
Monday - Friday 37.5 hours
Salary
This position is full time, youll receive an annual salary from £46,525 up to £52,507 per annum FTE - depending on experience.

Main duties of the job
As a Long Term Conditions Nurse, your responsibilities will vary. You will
- Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
- You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing
- A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.
- Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections
- Deliver continuity of care across health and social care agencies from courts, to custody and communiy.

Qualifications Required;
- Registered Nurse- General with relevant NMC Registration
- Advanced Nurse Prescriber/ Practitioner
- Educated to degree level
- Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
- Experience of assessing the needs of and working with individuals with complex health and social care needs.
