We have an exciting opportunity for a Release and Transfer Nurse to join our Primary Care team at HMP Pentonville, a category B/C Prison located in North London.

HMP Pentonville has a capacity for 1,100 primarily holds remand and convicted male prisoners. A significant portion of its population is on long-term remand. There is free parking on site. 

Hours

Monday - Friday - 37.5 hours per week.

Salary

This position is full time, youll receive an annual salary up to £50,000 per annum FTE - depending on experience.

Main duties of the job

As a Release and Transfer Nurse, your responsibilities will vary. You will

• Use your expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems.
• You will manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment. 
• Provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment.
• Be expected to work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment. 
• The Release and Transfer nurse will conduct a holistic patient assessment to include; Clinical, Psychological, Substance Abuse, Social, Educational, Training and Employment and Spiritual / Chaplaincy.

•  Registered Nurse with current NMC registration
• Must have prison experience - Essential
• Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness
• Experience demonstrating high levels of clinical expertise in decision making and judgement, enabling the development, monitoring and improvement of standard of care through supervision, clinical practice.
