Practice Plus Group are looking for clinicians for the new Unscheduled Care Coordination hub (UCCH) in Suffolk and North East Essex.
To be successful in this role, you will need to hold good knowledge of local urgent care, primary care and secondary care pathways and infrastructure. You will need to collaboratively work with the SNEE system partners to help deliver better quality of care for patients.

The service is currently operational 7 days a week, 0800hrs-2000hrs with opportunities for full or part-time work either salaried or bank. This is initially a fixed term contract for a year with the possibility of permanent employment at the end of this period.
Main duties of the job
- Working face to face as part of the multi-disciplinary UCCH team
- Reviewing low acuity cases held in the East of England Ambulance Service (EEAST) queue and identifying cases that may be better served by other Services
- Completing remote consultations for cases referred to the CAS
- Work with other members of the UCCH to identify and suggest service improvements with a focus on quality assurance and patient safety
- At times of reduced workload within UCCH, post-holders will be expected to work within the SNEE CAS and/or covid-19 anti-viral treatment service
- Act as a representative and ambassador for Practice Plus Group at all times
About us
- To work within the professional Code of Conduct as defined by the NMC or HCPC
- Current experience of working in urgent care, primary care or community care
- Educated to Masters level (or equivalent)
- Non-medical prescribing qualification with at least 6 months experience consulting and prescribing independently
- Ability to work within an MDT to make patient-focussed management decisions
- Evidence of regular engagement in evidence-based practice
- Willingness to undertake appropriate personal and professional development and keep up to date with mandatory training
- A clear understanding of remote consulting and appreciation of the challenges this can present
- An expectation to participate in regular performance review
