We have an exciting opportunity for an Advanced Nurse Practitioner to join our Primary Care team at HMP Thameside, a category B resettlement Prison located in Greenwich, South-East London.
In this role you will have the opportunity to provide the highest standards of healthcare to our patients, without having to compromise on efficiency and quality.
The working hours are: Monday to Friday - 9.00 to 17.00
This position is full time or part-time (working 3 days per week), youll receive an annual salary up to 61,000 per annum FTE - depending on experience.

Main duties of the job
As an Advanced Nurse Practitioner, your responsibilities will vary. You will
- deliver face to face services which will enable patients to obtain health information from the appropriate health care professional
- To work closely with the medical practitioners in the provision of quality, evidence-based care to patients within the health in justice setting
- be responsible for the assessment of care needs the development, implementation and evaluation of programmes of care, without supervision, and to teach other nursing and non-nursing staff

Qualifications & Experience required
- Registered professional with current registration (NMC)
- Postgraduate level ANP qualification (MSc)
- Recognised Nurse Prescribing Qualification
- Experience of working independently within Nurse Practitioner / Specialist role
- Understanding of the complexities of multi-disciplinary and multi-agency working

Practice Plus Group you will need expert clinical skills and demonstrate
judgement in assessing and managing wide ranging and complex patient
problems. You will provide clinical
leadership for the mental health team. This multi-disciplinary team is made up
of a variety of registered and non-registered professionals including; mental
health, and substance misuse practitioners. You will act as a role model
ensuring professional appearance and maintaining professional standards at all
times.
Supporting senior clinicians on site
you will be part of the daily leadership team, providing clinical input for all
patients. As a clinical expert in your field, you will work with the team to
ensure; patient safety, the management of complex cases and ensure the most up
to date treatment methodology is applied to patient care.

Provide
Outstanding evidence based care, listening and
acting on patient feedback so that care is personalised and informed by what
matters to them. You will also provide
assessment, diagnostics, treatment and continued management of patients with
acute and long term medical conditions.

Non-judgemental care by leading the team to deliver
care that is safe, high quality, patient centred whilst meeting CQC
requirements following our policies, procedures and always considering
safeguarding.
