Job summary
Practice Plus Group is building on its extensive experience in Primary Care. We deliver a new model of general practice called Practice Assist. Practice Assist provides additional remote capacity to GP Practices and prisons to give registered patients a full consultation by telephone or video. The clinical team also provide clinical admin support within the prison setting. Practice Assist currently provide support to 7 GP practices and 27 prisons.
We are currently looking for Prescribing Pharmacists to join the Practice Assist service. The post holder will lead, and be responsible for the pharmacy support to the PA+ service within Practice Assist.
The Prescribing Pharmacist will conduct medication optimisation clinics remotely without the patient being present, and prescribe for patients as deemed clinically appropriate within Practice Plus Group, local and national guidelines. Additionally, carry out long-term conditions review clinics in line with their specialist areas, signposting patients where appropriate.
You will also be working within the team management guidelines of roles and responsibilities for NPM.
The pharmacist must be available on any day Monday Friday, the clinics run 09:00 11:00 or 14:00 16:00.
Main duties of the job
The successful candidate will be a registered Pharmacist with the General Pharmaceutical Council as a Prescriber, ideally with a postgraduate clinical qualification. You should be a good communicator who can provide a holistic package of care to patients whilst working within a multi-agency team, recording accurate records of clinical advice. You should also have experience and excellent understanding of community pharmacy services.
Ideally the Prescribing Pharmacist will be able to carry out telephone consultations covering Diabetes, Hypertension and Asthma. Previous experience working for a prison would be advantageous although not essential.
