Job summary
You will report to the services County Manager. Under their guidance you will help the service to have effective governance systems and the continued development of Primary Care contracted services e.g. Shared Care and Pharmacy services.
Main duties of the job
- Audit oversight
Lead on the completion of serious incident investigation, CD LIN, Drug related Death Group and
CQC reporting and local incident review processes
Ensure coordinated and timely review of shared care and pharmacy Service Specifications, review of local SOPS relating to incident management and return of shared care and pharmacy agreements.
Work with Cranstoun Worcestershire Clinical Lead and the management team to organise primary care annual service reviews and training updates.
Lead on the development/production of primary care newsletters and incident learning updates.
Organise services monthly Incident Review Group (IRG) meetings ensuring that investigations have been submitted in a timely way with clear recommendations and actions set.
Line manage the Primary Care and Governance Assistant (PCGA) including provision of supervision, performance reviews and appraisals.

1. Duties and Responsibilities:
The post holder will lead on all aspects of quality and governance, PCN, shared care and pharmacy
agenda related to Cranstoun Worcestershire service delivery.
Comply with all Cranstoun policies and procedures.
Carry out duties which you are deemed competent to perform as directed by the County Manager.

2. Incident Management and Governance:
Lead on the management of the incident and feedback management system (Datix) within
Cranstoun Worcestershire including reporting, monitoring and investigating serious incidents
Identify and report on emerging trends and lead on lessons learned.
Ensure monthly local incident review groups take place.
Ensure incident monitoring systems are in place which allow for timely identification of any service
provision gaps and put measures in place to prevent breaches in policy, commissioner or
mandatory requirements.
Utilise Datix dashboards for performance monitoring, key themes and learning.
Ensure compliance with Statutory Duty of Candour disclosures and CQC mandatory notifications.
Produce regular service reports, analysis and learning/newsletters
Escalate Serious Incidents in line with the Cranstoun Group Policy.
Act as the main contact between the service and Cranstouns Serious Incident Review Group.
Lead on the development of compliance and monitoring systems and processes with Cranstoun
Worcestershire and commissioned primary care services.
Maintain skills and knowledge in relation to Datix incident management system.
Support both internal and external reviews as required.
Coordinate and act as Chair/deputy chair for Cranstoun Worcestershires monthly Incident Review
Group
Undertake investigations using Root Cause Analysis (RCA) methodology following incidents.
Liaise with the services management team in relation to quality assurance and improvements.
Provide coaching where required to staff members in the correct use of incident reporting systems.
3. External Requests for Information, Audit and Compliance
Complete all required CQC notifications and contribute and oversee the completion and submission
of other external reporting (e.g. Drug Related Death (DRD) and CDLIN incidents) for all incidents
that meet the criteria for submission.
Ensure that Coroner reports are completed to a high standard and submitted to the Coroners Office
within stipulated timescales.
Ensure that all requests and notification submission for information are submitted within specified
timescales
Ensure that queries and requests from pharmacies, GP surgeries and other bodies/stakeholders
e.g., Local Pharmacy Committee, Integrated Care Board are responded to in a timely way.
Ensure external monitoring systems/portals are monitored and updated within timescales stated.
Lead on the development of compliance and monitoring systems and processes for. Escalate
concerns to County Manager where appropriate.
4. Primary Care
Lead on shared care surgery and pharmacy visits and annual reviews.
Ensure any learning from surgery reviews is shared within the service and appropriate actions taken
for any identified issues.

Attend clinical prescribing meeting as required to support the delivery of a consistent approach
across specialist and shared care services.
Develop and maintain a database of primary and shared care provision across Worcestershire and
complete regular needs analysis to identify areas of need.
Review and consolidate shared care quarterly reports from Engagement and Recovery Workers and
Shared Care surgeries. Liaise with partners, including finance team, to resolve any queries.
Working with the County Manager, manage and update Service Level Agreements, policies, SOPs
etc. within shared care and primary care commissioned services.
Lead on shared care development e.g. onboarding and set up of a new surgery to shared care
scheme or expansion of service delivery
Produce Cranstoun Worcestershire newsletters/updates to pharmacy and shared care surgeries
In conjunction with the Clinical Lead develop and deliver annual shared care and pharmacy team
training days
Be the point of escalation for queries and concerns raised by primary care commissioned partners
and stakeholders including shared care surgeries, pharmacies, ICB and LPC.
Attend LPC meetings as required.
Oversee the arrangement of RCGP and shared care training updates. Ensure that best practice is
communicated to shared care GPs in a timely way.
Ensure relevant messages and development are communicated to pharmacies via
Pharmoutcomes.
Use Pharmoutcomes system to identify any themes or trends in community pharmacy provision
to inform service planning and budget management.
Liaise with pharmacies to ensure service specifications are in place for commissioned services.
Take responsibility for the development of compliance and monitoring systems and processes
within primary care.
Establish and chair forums for shared care workers and shared care GPs to discuss ideas and
concerns, share best practice in shared care and areas for development.
Work closely with operational leads regarding locality queries, changes or gaps in shared care.
Ensure shared care workers have received appropriate shared care induction including EMIS
access and training.
Be the point of contact to help resolve and respond to pharmacy and shared care queries. Lead
on shared care specific complaint investigation.
Oversee the community pharmacy needle and syringe programme provision to ensure
compliance with national and local requirements and respond to community safety issues such as
needle litter hotspots.
Support with the identification and transfer of clients between specialist prescribing and shared
care prescribing.
