Job summary

An exciting role within a modern and advancing integrated care setting, supporting the GP Practices of Farnham.

To provide enhanced nursing care and management to frail and housebound patients, delivering long-term condition support, holistic care, and improving patient outcomes in alignment with Quality and Outcomes Framework (QOF) targets. This includes proactive, high-quality care management to meet both individual patient needs and PCN-wide QOF indicators.

Main duties of the job

As an Advanced Practitioner (ANP/ACP) at Farnham Integrated Care Services, working as part of our Clinical team, you will provide an efficient same-day care service to our patients with a caring and respectful disposition. Working independently face to face or via telephone consultations, you will be accountable for delivering safe, clinical decision-making and expert care to a high standard. Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; prescribing and evaluation of their care.

We are looking for 4 to 7 sessions per week between the hours of 9.00am to 6:30pm Monday to Friday.

Confirmation of the ability to commute reliably or relocate to Farnham before starting work is required.

Advanced Nurse Practitioners Responsibilities:

Through telephone clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.

Formulate a management plan for the patient, using other sources of healthcare professionals (such as GP or pharmacists) if required to support diagnosis or next course of action.

Prescribe medication as required within your clinical competence.

Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate.

To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence.

Patient Assessment and Care Planning

Work within a multidisciplinary team (MDT) to assess, devise, and evaluate complex care plans for housebound and frail patients, ensuring that their unique health needs are met in alignment with PCN and QOF care goals.

Engage with patients, families, and caregivers to promote a patient-centred approach and ensure care aligns with both patient preferences and QOF targets.

Clinical Responsibilities

Conduct long-term condition reviews, including diabetes, cancer, learning disabilities, dementia, mental health, and blood pressure management, to support QOF performance and outcomes.

Deliver specialist care in line with level 7 qualifications, including advanced interventions and clinical decision-making for complex cases.

Use prescriptive authority (where qualified) to enhance medication management, especially in managing long-term conditions, with attention to QOF outcomes.

Provide patient and family education on managing health conditions, preventive care strategies, and self-management aligned with QOF indicators.

Candidates with experience or relevant job titles of Enhanced Nurse Practitioner, Advanced Paramedic Practitioner or Advanced Nurse Practitioner may be considered for this role.
