This full-time role (37.5 hours per week) offers a structured and supported pathway combining clinical practice, education, and service development, enabling progression to a substantive ACP role.

Main duties of the job
This developmental clinical role is designed
to support progression to Advanced Clinical Practitioner level. You will
undertake supervised clinical assessments, contribute to diagnosis and
management of patient conditions, and help develop personalised, evidence-based
care plans in partnership with patients and multidisciplinary teams.
Under appropriate supervision, you will
develop skills in assessment, clinical decision-making, and management of
patients with acute illness, long-term conditions, and complex needs. As your
competence grows, you will take on increasing responsibility in line with an
agreed learning and development plan.
This is not a standard solely patient-facing
role. You will also have the opportunity to contribute to service development,
quality improvement, and innovative ways of working, while gaining exposure to
leadership and system-wide practice through collaboration with divisional and
national teams. Flexibility, curiosity, and commitment to learning are
essential to developing safe, effective, and high-quality practice within a
multidisciplinary team.
About us
Our 4 practices are part of Modality
Partnership, one of the UKs largest GP super-partnerships, serving over
450,000 patients with a workforce of more than 1,500 staff. We are committed to
continuous improvement, innovation, and scalable solutions that enhance patient
care and staff experience across primary care.
In this role, you will be supported to develop
a broad portfolio of advanced clinical, leadership, and operational skills. You
will work alongside experienced clinicians and leaders, gaining insight into
service improvement and the future direction of advanced clinical practice.
Throughout your development, you will work in alignment with our CARE values,
helping to shape your career and the future of primary care delivery.
