We are looking
for a part-time Nurse Care Coordinator to join our AWC Division for 30 hours
per week. This is a varied and rewarding role focused on coordinating
personalised care and support planning, ensuring patients receive seamless,
holistic care across primary care and wider health and social care services.
This role is

Main duties of the job
This is a
dynamic, patient-focused role that ensures health and care planning is timely,
efficient, and centred around individual patient needs. You will bring together
information relating to a patients identified care and support requirements
and work collaboratively to explore and coordinate appropriate options within a
single personalised care and support plan.
The role
requires flexibility, innovation, and commitment and is not limited to standard
working patterns. You will work resourcefully as part of a multidisciplinary
team to ensure tasks are completed effectively and patient care remains
seamless.
Key
responsibilities include:
Coordinating personalised care and support planning for patients
Working collaboratively with healthcare professionals, patients, families,
and carers
Liaising with multiple stakeholders to meet identified patient outcomes
Providing advice, guidance, and signposting to appropriate services
Contributing to service improvement and patient experience initiatives
Working flexibly to support service delivery and patient needs
