Modality Partnership
Haydock Division has a part-time vacancy for a Call and Recall Care Coordinator
for 20 hours per week. The postholder will
be a high performing employee, working across Modality Haydock Division who
will proactively support patient care and divisional clinical targets, to help carry out administrative duties principally
patient call and recall, including data collection, monitoring, tracking and
reporting. We are seeking dedicated

Main duties of the job
The role is to
help coordinate and recall patients for their chronic disease management, and
to ensure that appointments are booked with the appropriate clinician so that
the correct care can be delivered to the patients. Working with the management
and clinical teams to ensure efficient use of clinical time through active
communications management of patients to help reduce DNAs.
The role is not
a regular 9am-5pm job; the job requires innovation, flexibility, and commitment
and the postholder will be required to work resourcefully as part of the team
to ensure tasks are completed.
