Modality Partnership
Birmingham Division has 2 full-time vacancies for a Call/Recall Care Coordinator. The postholder will be a high performing employee,
working across Modality Birmingham Division. The role involves coordinating
care plans and ensuring the well-being of individuals under our care. We are seeking dedicated postholders to join

is £23,875 per annum WTE rising to £24,110 following a successful probationary
period.

Main duties of the job
The role is to
help coordinate and recall patients for their chronic disease management, and
to ensure that appointments are booked with the appropriate clinician so that
the correct care can be delivered to the patients. Working with the management
and clinical teams to ensure efficient use of clinical time through active
communications management of patients to help reduce DNAs.
The role is not
a regular 9am-5pm job; the job requires innovation, flexibility, and commitment
and the postholder will be required to work resourcefully as part of the team
to ensure tasks are completed.
