Modality
Partnership AWC is looking for experienced nurses to be based at various sited within the division who would like to
consider a change in career to Practice Nursing. We would welcome applicants with at least 2
years post-registration experience in the following areas: Diabetes
management/respiratory management/womens health- gynaecology, contraception,
menopause. The postholder will be a high performing nurse, who is willing to
further train in areas of Practice Nursing; working across Modality AWC Division
and responsible for the delivery of general practice nursing to the whole
practice population. We are seeking

Main duties of the job
The role is
both the delivery of evidence-based practice for patients presenting with a
chronic condition, and the provision of preventative health care to the
practice population. for the care delivered to their patients, demonstrating
critical thinking and skills in clinical decision-making in the management of
patients.
The role
requires innovation, flexibility, and commitment and the postholder will be
required to work resourcefully as part of the team to ensure tasks are
completed.
