Job summary
Modality Partnership AWC Division is expanding and is offering equivalent of 3 full-time vacancies for Advanced Clinical Practitioners. The postholder will be passionate about patient care, working at one of our sites across Airedale & Craven, that are all within a 10 mile radius of our local acute trust, base can be flexible depending on your requirements and collaborations. We are seeking qualified ACPs who will contribute to innovation within the partnership in the way care is delivered, and who will embrace our partnership values: Commitment, Accountability, Respect and Excellence (CARE).
Airedale sites include Holycroft Surgery, Farfield Group Practice, Kilmeny Group Medical Practice, Long Lee surgery, Haworth Medical Practice and Oakworth Medical Practice
Craven sites include Cross Hills Group Practice, Silsden Surgery, Steeton Surgery and Fisher Medical Centre-addresses below.
This job is suitable for individuals who are compassionate, patient focused and have a real passion for primary care, and working within a larger GP super partnership, who are dedicated to engaging with health and social services to improve patient care.

Main duties of the job
The role is a patient centered, all-rounded clinical ACP position. The postholder will need to confidently evidence key skills such as excellent communication skills, clinical skills and empowering patients to manage their individual health conditions. Whilst working independently and using their own initiative for part of the role, the postholder will also work in collaboration with their local practice team, community teams and will engage the use of the associated roles that are be attached to the practice.
A background of integrated health and social care planning in partnership with the individual, carers, relatives and health care professionals, based on assessment of medical, nursing and social care needs will be beneficial. Furthermore, specific interest in care homes and the elderly is ideal. The postholder will be able to demonstrate ability to act as an autonomous advanced practitioner providing a comprehensive clinical assessment service to patients/carers/families resulting in safe, appropriate clinical decisions. Supervision by General Practitioners in practice is readily available as needed and is encouraged where support is needed.
