Make a real difference to people living with longterm lung conditions.
You will work alongside Respiratory Physiotherapists and Nurses to support the delivery of safe, effective care for people living with long-term lung conditions such as COPD and bronchiectasis.Using your coaching skills and knowledge of exercise, youll help patients build confidence, improve physical ability, and achieve their personal rehabilitation goals.
Base
The role will be based at St Martins Hospital, Bath (BA2 5RP), with travel required across the Bath and North East Somerset (BaNES) area, and occasional travel to Swindon and Wiltshire.
Main duties of the job
- Set up and deliver pulmonary rehabilitation exercise programmes across a range of community settings, including potential virtual/online classes
- Actively support the booking and coordination of patient appointments for 1:1 assessments and group classes
- Carry out delegated tasks from the Team Leader or Senior Physiotherapist
- Manage and prioritise your own caseload with professional autonomy, supported by senior colleagues
- Support individuals to achieve personcentred goals, monitoring progress and adjusting interventions as needed
- Plan, progress, and tailor exercise programmes for participants attending pulmonary rehabilitation classes
- Use initiative and sound judgement to develop exercise programmes and support participants through to completion
- Evaluate outcomes and prepare discharge summaries
- Liaise with external agencies where appropriate to support patient care
- Support service promotion and engage with stakeholders as part of the wider team

- Experience working in a healthcare rehabilitation setting, either hospital or communitybased
- Strong interpersonal and communication skills
- Excellent organisational and timemanagement skills
- Proven ability to work effectively as part of a team
- Good literacy and IT skills, including Microsoft Word, Excel, PowerPoint and Outlook
- Confidence using digital platforms such as Microsoft Teams to support online sessions
- Experience supporting and educating individuals and groups, including patients, colleagues, and professionals
- Ability to work independently using initiative, with appropriate supervision
- Experience of partnership working and building relationships with external organisations
- Understanding of relevant health and care legislation
- A valid UK driving licence and access to a car for work purposes

- Completion of training or courses related to health, rehabilitation, or exercise
- Experience communicating information to a wide range of colleagues
