Job summary
Make a real difference where it matters most. As part of our communitybased Heart Failure Team, youll play a vital role in helping patients regain confidence and exercise safely in their own homes or local community venues. Youll work closely with colleagues to support people across Wiltshire, with some travel across the wider BSW area as needed - bringing highquality care directly to those who need it most.
Main duties of the job
- Support the delivery of cardiac rehabilitation for heart failure patients under the supervision of the Heart Failure Cardiac Rehabilitation Nurse
- Work autonomously while recognising when to seek clinical support or escalate complex cases
- Undertake independent home visits, monitoring patient progress and referring concerns appropriately
- Deliver safe rehabilitation support in community settings (e.g. leisure centres), following loneworking policies and completing risk assessments
- Communicate effectively with patients and carers using clear, motivating, and adaptable approaches to maximise rehabilitation outcomes
- Maintain accurate electronic patient records, including data entry for local and national cardiac rehabilitation audits
- Attend and actively contribute to team meetings
- Refer or signpost patients to other relevant services as needed
- Complete all mandatory training in line with Trust requirements
- Undertake additional duties appropriate to the role as delegated by senior staff

- Experience working in a health or social care environment
- Excellent communication skills with the ability to motivate and reassure patients
- Ability to work autonomously and know when to seek clinical support
- Willingness to develop clinical skills relevant to cardiac rehabilitation
- Ability to follow standard operating procedures and organisational policies
- Confident using IT systems, including maintaining accurate patient records
- Willing to undertake Heart Failure Manual Training
- Full UK driving licence and access to a roadworthy vehicle
- Ability to work in line with lone working policies and community-based risk assessments
Desirable
- Experience in cardiac rehabilitation or rehabilitation therapy
- Experience supporting service assessments or reviews
- Working towards, or willing to work towards, an NVQ Level 3 or equivalent
