You will be providing high quality, personalised care in the place where children feel safest and most comfortable their own homes and community settings.
In this vital role, you will work closely with families and clinical teams to deliver safe, sensitive, child centred care. You'll need to be flexible, as the needs of each child can change but the reward is knowing your support directly enhances their wellbeing, comfort, and independence.

Main duties of the job
- Deliver safe, effective care to children in the community as delegated by the Named Nurse.
- Identify safeguarding concerns and follow appropriate procedures.
- Maintain competencies in specific clinical skills and understand emergency procedures, including escalation and resuscitation.
- Support children's learning, development, and wellbeing in partnership with families and multidisciplinary colleagues.
- Help create a safe, stimulating environment and monitor and record each child's condition, taking action where needed.
- Organise day to day activities and attend mandatory training, supervision, and team sessions.
- Complete the HCRG Care Certificate within 3 months (if required).
- Follow infection control, confidentiality, and health & safety procedures.
- Communicate effectively with children, families, and professionals, adapting your approach where communication barriers exist.
- Participate in team meetings, maintain accurate records, and contribute to a supportive team culture by sharing information and raising concerns.
This is a 24/7 service, and the role includes night shifts and weekend work on a rotational basis.
