Job summary
Make a real difference in stroke recovery.
Join our interdisciplinary Community Stroke Team, where youll work closely with therapists to help assess, plan, and deliver personalised rehabilitation programmes that support people to regain independence and rebuild their lives.
Main duties of the job
- Carry out basic therapeutic treatments and manage your own caseload, delivering highquality support to people receiving stroke rehabilitation in the community.
- Work under the guidance of Physiotherapists, Occupational Therapists, and Speech & Language Therapists to ensure an efficient, effective daytoday service.
- Contribute to the teams shared goal of helping patients regain maximum physical, mental, and social independence, while supporting carers in their role. Assist with implementing individual treatment programmes as directed by the clinical team.
- Work flexibly as required, which may include weekend shifts.
- Support essential administrative taskssuch as writing letters and ordering suppliesto help the service run smoothly.
- Promote care in the most appropriate setting, helping prevent unnecessary hospital admissions whenever possible.
- Ensure patients and carers remain central to all decisionmaking and rehabilitation planning.
- Take ownership of your ongoing learning, engaging actively in CPD and development opportunities.
