Job summary
Our work is crucial to the community as we aim to prevent unnecessary hospital submission and support hospital discharges. In this role, you will care for patients by delivering our high service standards, sharing best practice, and actively helping to develop and improve the way we work.
We empower our staff to work as autonomous staff members, and as part of a multidisciplinary team, with the provision of ipads / tablets to promote remote working and enable staff to be out in the community supporting patients within their own home.
Main duties of the job
- To assist in the provision of a high standard of planned individualized, holistic rehabilitation programmes for patients in the community, under the care of the Urgent Care Response and Intermediate Care Team, including recuperative and palliative care.
- To support the management and discharge of patients as part of the rapid response team. Will be required to work in the community in support of this.
- Being prepared to work over seven days per week in order to meet service needs.
- Will support general practice in early identification and anticipatory care planning for all patients identified at risk of admission to hospital in order that inappropriate admissions are avoided.
- To assist in providing nursing/therapy care as delegated by the Lead Therapist/Team Sister within patients homes. This includes providing short term personal care prior to a care package commencing e.g., washing, dressing, meeting all hygiene needs.
- To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
- To participate in and support qualified team members in health promotion activities
- To provide clerical and administrative support to the team as delegated by the Lead Therapist/Team Sister
