We are looking for a dedicated and proactiveHealth Family Practitionerto join our Lancashire Young People & Healthy Families Service.In this role, you will work under the direction of our Health Visitors (SCPHNHV), School Nurses (SCPHNSN) and Staff Nurses, supporting preschool and primaryaged children and their families.Your knowledge of child development and commitment to improving health outcomes will help deliver key elements of the Healthy Child Programme and targeted interventions based on need.
You will provide care across a range of community settingsincluding lone working in family homesand will be expected to uphold HCRG Care Groups values by delivering excellent, compassionate care.
Main duties of the job
What Youll Do:
- Deliver delegatedmandated health reviews, including Pre1Year and 22.5Year developmental checks, identifying additional needs and referring into specialist services when required.
- Work confidently incommunity settings, using your own initiative to support families.
- Providetargeted supportaligned to your competencies, such as behaviour management, infant feeding, and other delegated care packages.
- FacilitateHealthy Family DropIn sessions (Child Health Clinics)at designated community sites.
- Support the delivery ofBump, Birth and Beyondsessions.
- Act as alink to nursery settings, supporting early years staff and families.
- Ensureservice user voicesare captured and fed into service development.
- Contribute to and supportrisk assessments, ensuring safe working practices in line with health and safety and organisational policies.
- Promote engagement with children, young people and familiesacting as anadvocatefor their views and choices.
- Ensure inclusivity by using appropriate communication tools and services (e.g., language link workers, interpreters).
- Participate in theCAF process, attend meetings, and act as lead professional when appropriate.
- Maintainaccurate, comprehensive, contemporaneous records, including data entry.
