Main duties of the job
As anAdvanced Clinical Practitioner,you will report to the Consultant Practitioner.
- Deliver an autonomous ACP service in Community Hospital inpatient settings, meeting national and organisational standards.
- Complete advanced assessments, diagnose, initiate treatment, arrange investigations and coordinate care efficiently.
- Support patient centred decisions, working closely with patients, carers and professionals.
- Provide strong clinical leadership, uphold NMC standards and proactively manage clinical and staffing issues.
- Prevent unnecessary transfers to acute care and support safe discharge planning with Community Team Key Workers.
- Ensure care remains patient focused, resources are used effectively and contribute to budget oversight.
- Respond promptly to staff concerns, incidents, complaints and safeguarding issues, ensuring learning and improvement.
- Lead and participate in service development aligned with national guidelines (e.g., NICE, NSF).
- Maintain compliance with IG, health, safety and security standards.
- Oversee and act on clinical audit findings, promoting continuous improvement.
- Foster a learning environment, ensuring staff complete mandatory training, appraisals and clinical supervision.
- Support research, audit and innovation, sharing outcomes and embedding changes into practice.
- Monitor performance, contribute to risk assessments and support professional, clinical and quality meetings.
- Provide cover for fellow ACPs when required.
