Assistant Practitioner -Learning Disability & Autism Service
Were excited to offer a great opportunity for an Assistant Practitioner to join our forward thinking Learning Disability and Autism Service. This role is perfect for someone with experience supporting people with a learning disability who is ready to build their skills within a specialist team.
As part of our service, youll work directly with individuals who may have complex health needs, behaviour that challenges, and/or mental health needs. Youll play an important role in assessing needs, promoting independence, and ensuring that people receive high quality, person centred support.
Please note that a full driving licence and access to your own vehicle are essential for this role.

Main duties of the job
- Provide person centred, evidence based care to individuals, working under the indirect supervision of a registered professional and within agreed guidelines.
- Carry out specific clinical tasks that you have been trained and assessed as competent to perform.
- Support registered practitioners and colleagues in delivering high quality care.
- Work in partnership with individuals, their families and carers to help develop personalised care plans.
- Contribute to care planning, with oversight from a registered practitioner.
- Encourage and promote independence in the people you support.
- Promote health, wellbeing, and healthy lifestyles when working with individuals with learning disabilities and their carers.
- Carry out routine tasks with appropriate supervision from qualified staff.
- Follow the guidance and direction of the multi professional team to support each persons health, independence, and rehabilitation.
- Maintain, monitor and promote a safe environment for individuals, families and staff.
