Main duties of the job
- Support patients with daytoday care, following their individual care plans.
- Help people stay independent, safe, and well in a community setting.
- Work as part of a multiprofessional team to deliver highquality, personcentred care.
- Assist with rehabilitation activities and help reduce unnecessary hospital admissions.
- Monitor patients wellbeing and report any changes to registered staff.
- Support patients with medication when authorised by a qualified practitioner.
- Communicate clearly with patients and carers, adapting for language or hearing needs.
- Maintain accurate records of each patients needs, wishes, and progress.
- Follow infectioncontrol procedures and organisational policies at all times.
- Provide compassionate care, including support for terminally ill patients and their families.
- Always obtain patient consent before providing care or treatment.

Job responsibilities
As a Healthcare Support Worker, youll be part of our valued team based on Forest Ward at the Swindon Intermediate Care Centre. You will feel valued within HCRG Care Group, receiving access to exclusive rewards and benefits including:

What We're Looking For
- Good general education and willingness to complete NVQ Level 3 in Health & Social Care if not already achieved.
- Experience working under indirect supervision and supporting colleagues.
- A strong team player with a patient focused approach.
- Awareness of personal limitations and willingness to seek support when needed.
- Basic typing and data entry skills.
- Ability to safely assist with moving and handling patients using appropriate equipment.
- Ability to carry out delegated nursing or therapy tasks.
- A full driving licence and access to a car for work purposes.
- Flexibility to work across other HCRG Care Group services when required.
- Self motivated, organised, and driven to achieve high standards.
- Good time management skills.
- Food Hygiene certificate or a willingness to complete training.
